Gear knob threaded adapter removal

Hi, I'm new!
Sorry if this is a silly question or it's been done before.

I have purchased a new gear knob for my type 9 box, 3/8" thread. When I went to fit it (I ordered it while away and didn't check) and I have an adapter screwed on top for the current plastic knob.

I need to remove this but there's nothing to get a grip of. I thought maybe put two nuts locked together on the adapter and use a spanner to undo it but don't know what size but it's a very course thread. And griping the gear stick itself is a bit dubious too. It seems to be stuck fast, I've used penetrating fluid but hasn't made any difference. 
Has anyone had any experience of this, any ideas welcome!

Upper bit: self-grip wench, or two nuts as you suggest.
Lower bit: self-grip wench
Assistance: soak in penetrating oil, as already done, NOT ordinary WD-40, and if that doesn't work then local heat on the female end of the adaptor.

Then tonk with a mallet and confidence that it will work this time!
